I have never seen this happen before but this grande sent up a thick spike (as can be seen in the pic ). On top of this thick green spike two leaves have developed and a further small spike with one bud ( see pic ). In the middle of the two leaves further leaf growth is beginning.
Hope I've described this in an understandable way and would appreciate your thoughts.

I have seen that happen before. The smaller bud was or is a premature part of that spike that has emerged.

That bud looks dried. How many buds are there?

I agree with what you said but it could also be branching off from the spike.

To me, that looks like the spike with a bud coming out. The spike should keep growing upwards and more buds will come out like that.

That bud looks dried. How many buds are there?

There is only one bud as you can see on the pic but it is in perfect condition. Sorry if the pic makes it look otherwise--thanks for looking and for your comments.

Yes I have also seen that happen to both my Paphs and Phrags.
