I have had this mature PEOY for several years...from the day it arrived (from a nursery whose owners are now retired) the plant had these marks all over the old leaves. Figured it was old damage, but now it is appearing on new leaves as well. I'm beginning to think this plant is doomed.

viral? bacterial?

Im not sure if its diseased or not but if the new leaves have the marks also then I would get rid of it. Not worth wiping out your whole collection. Lets see what others have to say.

I reckon thats a fungal disease. Urgent treatment required with your preferred method.

It is still a beautiful plant and I would not give up on it yet. Maybe a genetic trait, maybe fungal. I would take it out of the pot and give it a physan bath for an hour or so. Let it dry thoroughly and repot it with a stable and airy mix with no fertilization. Keep it from getting moist unless to water and make sure it drys well by nightfall. As a precaution, yes I would still isolate it from the rest of my plants, especially when watering.
